The Licensed Vocational Nurse (LVN/LPN) provides skilled nursing care to residents in accordance with physician orders, facility policies, and regulatory requirements. The LVN focuses on resident assessment, administration of medications and treatments, and collaboration with the interdisciplinary team to ensure quality, safe, and compassionate care.
Reporting:
This position reports directly to the Director of Nursing and collaborates with the interdisciplinary care team.
- Assess residents’ physical and mental health status and contribute to individualized care planning.
- Administer medications and treatments safely according to physician orders and facility protocols.
- Monitor, record, and communicate changes in residents’ conditions to the care team.
- Educate residents and families on care plans, health management, and preventive strategies.
- Participate in interdisciplinary care planning and resident care conferences.
- Maintain accurate and timely documentation of resident care, interventions, and outcomes.
- Ensure compliance with infection control practices, safety standards, and resident rights.
- Provide mentorship and guidance to nursing assistants and other support staff as appropriate.
- Participate in facility quality improvement initiatives, in‑services, and continuing education programs.
Graduate of an accredited vocational nursing program.
Current, active Washington State LVN license.
Experience in skilled nursing, long-term care, or rehabilitation settings preferred, but new graduates are welcome to apply.
Qualifications and Skills- Strong communication, interpersonal, and critical thinking skills.
- Ability to follow care plans, collaborate with team members, and provide resident‑centered care.
- Compassionate, patient, and committed to quality resident care.
- Physical ability to safely assist residents and perform nursing duties including lifting, standing, and walking for extended periods.
Skilled Nursing Facility environment with direct interaction with residents, staff, and visitors. May involve walking, standing, lifting, and exposure to bodily fluids and infectious materials. Must be able to cope with the stresses of working in a 24‑hour care environment serving elderly and medically complex residents.
