Home Health and Hospice RN

Job Description

We have a need for a weekend Home Hospice RN for the Fairfield County CT area. The schedule is 24 hours, AM shift. We would welcome the chance to speak with you about this opportunity.

• Must be skilled in pain and symptom management.
• Able to triage patient needs and make patient visits as appropriate.
• Provide skilled and supportive nursing care to patients and families.
• Perform assessment of patient and family through observation, interviews, and analysis of records.
• Respond to physical, emotional, psychosocial, and environmental needs.
• Identify patient and family needs for other home health care services and make referrals through agency-designated channels.
• Identify signs and symptoms of the disease and explain diagnosis and the nature of palliative care to the patient and family as appropriate.
• Utilize appropriate nursing interventions to alleviate pain and discomfort in accordance with the plan of care.
• Support decision making by the patient and primary caregivers.

• Minimum of two years of nursing experience, preferably in medical/surgical nursing.
• Valid Clean Driver’s License required.
• Home Health Care and/or Hospice / End of life care experience preferred.
• Active client contact within the past three years, preferred.
• Certified Hospice and Palliative Care Nurse (CHPN) preferred.

Hourly rate: $40.00 - $44.00.
