Job Description- The LPN provides quality nursing care and services to residents in a respectful and dignified manner.
- The LPN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with physician orders and resident choices.
- Executes the medication management plan, including periodic review of documentation and communication with the physician when changes are needed or outcomes are not appropriate.
- Performs continuous assessment of resident needs and updates appropriate individuals regarding care, services, and changes in condition.
- Implements measures to ensure resident wellness and safety, documenting those measures appropriately.
- Works in a team fashion to meet resident needs.
- Responds to emergencies promptly, calmly, and competently.
- Reports incidents and changes in resident condition to the Healthcare Director with the appropriate documentation.
- Actively participates in all activities, including community events.
- Assists with the delivery of meals in the dining room, monitors resident acceptance of diets, and ensures adequate nutrition.
- Maintains high standards of record‑keeping, resident, and staff documentation.
- Establishes and maintains a positive working relationship with all departments.
- Identifies risk factors for residents and initiates interventions, documenting those interventions appropriately.
- Provides a thorough report of resident status to the incoming nursing team at each shift change and in the daily log.
- Provides motivation and direction to CNAs/RMAs.
- Attends required trainings, CEUs, and staff meetings.
- Adheres to and implements all policies and procedures.
- Practices proper body mechanics and safe resident‑transferring techniques.
- Abides by attendance policies, reporting to work on time and prepared to begin duties.
- Completes all other assigned duties.
- Current LPN license in good standing with the Board of Nursing in the state where the community is located.
- Experience working with the senior population, preferably in assisted living or long‑term care.
- Team player who works well with others and has experience supervising staff.
- Critical thinking skills and effective verbal and written communication abilities.
- Demonstrated ability to manage time efficiently with strong organizational skills.
- Good physical condition; able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
- Willingness to work an 8–16 hour flexible schedule that includes weekends and holidays.
- Attendance at all required trainings, CEUs, and staff meetings.
- Certified or willing to take and pass the certification class required by the state in which employed.
