What You'll Be Doing

As aBilingual Patient Support Specialist (LPN), you'll provide hands-on clinical support while helping patients access the care and resources they need.

Responsibilities Include:

  • Deliver patient-centered nursing care in an ambulatory primary care setting
  • Conduct proactive patient outreach and engagement to improve continuity of care
  • Coordinate care plans and assist patients in navigating healthcare services
  • Respond to patient phone calls and electronic messages while escalating concerns when needed
  • Perform clinical testing and nursing procedures within LPN scope of practice
  • Support primary care and specialty care programs
  • Maintain exam rooms, clinical supplies, and medical equipment
  • Accurately document patient care in electronic health records (EHR)
  • Participate in population health and quality improvement initiatives, including UDS and HEDIS measures
  • Ensure compliance with HIPAA, OSHA, CLIA, HRSA, and Joint Commission standards
  • Collaborate with providers and interdisciplinary care teams to deliver exceptional patient outcomes

Qualifications

Required:

  • Graduate of an accredited Licensed Practical Nurse (LPN) Program
  • Active Connecticut LPN License
  • Current CPR/BLS Certification
  • 2-4 years of clinical experience in:
    • Primary Care
    • Ambulatory Care
    • Acute Care
  • Strong care coordination and patient communication skills
  • Experience with Electronic Health Records (Epic experience preferred)
  • Ability to thrive in a fast-paced healthcare environment
  • Bilingual in Spanish and English

Job Type & Location

This is a Contract to Hire position based out of Greenwich, CT.

Pay and Benefits

The pay range for this position is $31.25 - $36.00/hr.
