Job Type:Permanent / Direct Hire
Setting:Home Health
Location:Bellevue & Medina, WA / King County
Schedule:Full-time, Monday-Friday; on-call as assigned
Pay and benefits:$50-$62/hr DOE + mileage reimbursement; medical, dental, vision & life insurance, 401(k) match, 16 days PTO + holidays, paid sick leave & more
Company overview:Family-focused home health organization providing compassionate, patient-centered care to individuals in their homes throughout King County.
Job Description:- Provide skilled nursing care to patients in their homes according to physician orders and individualized plans of care.
- Complete initial and ongoing nursing assessments and develop/revise plans of care.
- Monitor patient conditions, communicate changes to physicians and coordinate needed services.
- Educate patients and families regarding care, treatment and health needs.
- Complete clinical documentation, OASIS assessments and required Medicare documentation.
- Coordinate care with PT, OT, SLP, social workers and other healthcare professionals.
- Supervise and provide guidance to LPNs and CNAs as assigned.
- Participate in patient care conferences and quality improvement activities.
- Current Washington State RN license.
- 1+ year of nursing experience preferred.
- Home health experience preferred.
- Ability to travel throughout assigned areas of King County.
- Valid driver's license and reliable, insured vehicle.
