Liaison Nurse

About This Role

The Liaison Nurse at Kaiser Permanente will work in two settings, focusing on discharge planning and follow-up care for patients. This role is ideal for experienced nurses who excel in collaborative care and patient advocacy.

• Pay: $49 - $82/hour

• Facility: KPWA Bellevue Medical Center
• Schedule:
  
  Full-time, Variable Mon-Fri, 10AM-8:30PM, Every other weekend rotation, Rotating holidays
• Sign-On Bonus: $5,000 for eligible external hires

• Ensure patients referred to case management meet established criteria.
• Assess patients' physical, mental, financial, and psychosocial status.
• Develop patient-centered case management plans with specific goals and time frames.
• Coordinate and implement interventions using evidence-based guidelines.
• Collaborate with healthcare professionals regarding the plan of care.
• Monitor and evaluate patient responses to therapeutic interventions.
• Facilitate referrals for home health care, long-term care, and hospice services.
• Participate in care conferences for patients with complex care needs.
• Collect data to evaluate the effects of care coordination on quality outcomes.
• Assist patients in understanding and complying with their medical treatment plans.

• Education:
  
  Associate's Degree (ADN) required
• Experience:
  
  Experienced level required
• Licenses &
  
  Certifications:
  
  Current RN License in Washington
