About This Role

The RN Case Manager Liaison Nurse will provide and coordinate nursing care for members in a designated clinic setting, focusing on chronic condition management and optimal patient health outcomes. This role is ideal for experienced nurses who thrive in collaborative environments and are dedicated to patient advocacy.

• Pay: $49 - $82/hour
• Location: Seattle, WA
• Schedule: Full Time, Day shift

• Ensure patients referred to case management meet established criteria.
• Assess patients' physical, mental, financial, and psychosocial status.
• Develop patient-centered case management plans with specific goals and outcomes.
• Coordinate and implement interventions using evidence-based guidelines.
• Conduct ongoing assessments of progress against original goals.
• Collaborate with healthcare professionals regarding the plan of care.
• Monitor and evaluate patient responses to therapeutic interventions.
• Facilitate referrals for home health care, long-term care, and hospice.
• Collect data to evaluate the effects of care coordination on quality outcomes.
• Assist patients in understanding and complying with their medical treatment plans.

• Associate's Degree (ADN) required

• Experienced level required

• Current RN License in Washington State
