• Conduct annual wellness visits and manage high-risk patients, primarily at the patient's residence, supplemented by telehealth.
• Thoroughly document medical histories, symptoms, and medication information.
• Administer various tests and screenings.
• Collaborate with our Primary Care team to ensure holistic patient care.
• Ensure timely completion of patient records, with access to electronic medical records and portals.
• Provide patient education on preventive care and prescribed treatments.
• Maintain strict confidentiality in line with HIPAA regulations.

This is a full-time Nurse Practitioner opportunity in clinic. The position requires flexibility across regions to provide coverage as needed to accommodate scheduling changes. We seek candidates passionate about delivering top-quality care, expanding access to comprehensive healthcare, and embracing a holistic approach.

• Nurse Practitioner certification from ANCC or AANP.
• 2+ years clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care. Home care experience is a plus.
• Working knowledge of chronic care management and medication options.
• Current DEA licensure / Prescriptive Authority post-hire.
• Active CPR/BLS Certification.
• Bilingual preferred – English and Spanish.
