Avalon Care Center Bellingham is seeking an Admissions Nurse to join our team!
Full‑time, Monday – Friday, 11:30 am – 8:00 pm.
Compensation: $43.50 – $48.05 per hour.

Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.
Responsibilities- Complete admission assessments for incoming residents, including physical and clinical evaluations.
- Coordinate admissions to ensure a smooth transition into the facility.
- Review hospital records, physician orders, medications, and treatment plans prior to admission.
- Collaborate with physicians and interdisciplinary team members to support continuity of care.
- Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
- Educate residents and families regarding admission procedures, care plans, and facility policies.
- Assess residents for appropriate level of care and identify immediate clinical or safety concerns.
- Coordinate room assignments, equipment, and resident care needs upon admission.
- Maintain compliance with state, federal, facility, and HIPAA regulations.
- Assist with resident transfers and discharge coordination as needed.
- Preferably 1–3 years of clinical nursing experience, including at least two years in long‑term care and two years of successful experience in administration, consultation, or involvement in the survey process.
- Must have an LPN license in good standing.
