In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem -Bellingham, WA

Join us as a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, engaging patients in their homes and bridging gaps in care.

The visit lasts about 45 minutes and includes a medication review, medical history, physical evaluation, and, if ordered, additional diagnostic tests. You will use an iPad and an intuitive clinical workflow. You are not required to prescribe medicine, order lab tests or alter treatment regimens.

• Be flexible to travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Provide Diagnostic & Preventive Services products (e.g., spirometry, DEE)

Your primary workplace is patients’ private residences. Candidates must safely perform duties in homes that may contain pets, secondhand smoke or allergens.

• Hourly base rate plus incentive structure per completed visit
• Malpractice insurance coverage during Signify Health-related activities

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Multi-state licenses (preferred)
• Ability to cover Bellingham and surrounding area

Typical pay range: $46.03 - $99.14 per hour

Part-time position eligible for benefits such as dental, vision, wellness resources, employee discounts, voluntary benefits and other programs based on hours worked and eligibility.
