WHAT YOU WILL DO:

Provides case management services to participants in Opportunity Council’s Housing Programs. Provides leadership and guidance to case management staff around housing programs, short and long term services and assistance. Ensures implementation of Motivational Interviewing, Housing First, and Harm Reduction case management model. Assists in managing housing data, client file tracking and compliance, ensuring data quality and timely client file reviews. Provides leadership and direction on issues affecting low-income populations and works closely with agency service centers and community partners to ensure coordinated service delivery.

LANGUAGE REQUIREMENT:

To effectively serve our diverse community, this position requires fluency and proficiency in English and Spanish.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
  • Provides housing-related case management services using Harm Reduction and Advance Motivational Interviewing model to households participating in housing programs, including assessment, resource and referrals, planning, goal setting, support, and evaluation.
  • Works with housing case management staff, providing leadership, guidance, and direction in program and service delivery.
  • Performs client file and data quality reviews for housing programs.
  • Maintains up-to-date knowledge of major systems and resources that low-income households encounter and utilize.
  • Maintains current, accurate and confidential records and provides reporting and tracking information for data management systems.
  • Documents and reports specific client activities for Medicaid Billing in applicable programs.
  • Collaborates with other agencies and service providers to develop service delivery plans.
  • Conducts home visits both on-site and off-site.
  • Ensures compliance with all federal, state, agency, and/or funder guidelines.
  • Assists in program development work.
BILINGUAL POSITIONS ONLY:

Specialize in providing services to clients and families who speak a language other than English as a primary language. May occasionally be asked to support with translating/interpreting a language other than English to assist potential or current clients.

OUR IDEAL CANDIDATE WILL HAVE:
  • Bachelor’s degree in Human Services, Psychology, Sociology, or related field, or equivalent year-for-year experience.
  • Minimum 6 months of related experience.
  • Experience in case management services with demonstrated competency in Motivational Interviewing, Harm Reduction, file management, and demonstrated ability to effectively support persons with complex needs achieve housing stability.
  • Experience working with people living with mental illness.
  • Experience working with people experiencing homelessness.
  • Experience working with persons experiencing substance use disorder.
