We are seeking a skilled and compassionate Registered Nurse (Home Health) to join our client’s expanding team in Bellwood, Illinois. This role involves delivering personalized nursing care to patients in their homes, promoting wellness, recovery, and independence. The ideal candidate will bring strong clinical skills, attention to detail, and a patient‑centered approach to care.

• Provide direct nursing care to patients in their homes based on individualized care plans
• Conduct comprehensive assessments and develop or update care plans as needed
• Monitor patient progress and communicate findings to the healthcare team
• Educate patients and caregivers on disease management, medication use, and home safety
• Maintain accurate, timely documentation in compliance with professional and regulatory standards
• Collaborate with interdisciplinary care teams to ensure quality and continuity of care
• Participate in team meetings, training, and case conferences as needed

• Monday–Friday, day shift
• Includes one weekend per month, 2–3 on‑call days per month, and two holidays per year
• This is a Pay Per Visit position

• Local travel to patient homes within the Bellwood area

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• At least 1 year of professional RN experience (home health preferred)
• Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association
• Valid driver’s license, reliable transportation, and auto insurance

• Previous home health or community‑based nursing experience
• Strong organizational and clinical documentation skills
