The Speech‑Language Pathologist is responsible for evaluation, diagnosis, and treatment of various communicative, cognitive, and swallowing disorders.
Responsibilities- Responds to referral and schedules patient according to department policies and procedures.
- Performs screenings and diagnostic evaluations.
- Interprets findings of evaluations and diagnoses communicative, cognitive, and swallowing disorders.
- Establishes and executes appropriate treatment plans.
- Refers patients to other professionals and agencies when indicated.
- Informs, counsels and consults with patient’s family, case managers, industries, etc. and other health care workers.
- Documents patient data (evaluation, treatment and progress notes, D/C summary, charges) and department statistics according to department policies and procedures.
- Develops and executes appropriate discharge plans.
- Participates in team conferences and discharge planning with departmental staff and/or members of the interdisciplinary team.
- Participates in performance improvements, community and staff education programs.
- Shares in supervision of student interns of Speech Language Pathology programs, shadow students.
- Performs the necessary functions to prepare, maintain, and clean up the work area to ensure cleanliness and safety.
- Maintains the protected health information of patients in their care.
- Documentation is required and is in accordance with professional, regulatory, and institutional guidelines.
- Strong communication skills and a desire to strive for excellence are essential components of this position.
- Respectfully interacts with a wide variety of internal and external constituents, including (but are not limited to) team members, staff and physicians, local companies, patients, families, vendors, and all insurance representatives.
- Provide coverage to other Physical Medicine locations and settings as necessary based on patient care/departmental needs.
- Work schedules are determined to meet patient care/department needs and shifts may be required on weekends and holidays.
- Master's Degree in Speech Language Pathology from an accredited school or higher degree equivalent.
- Certificate of Clinical Competency in Speech Language Pathology American Heart Association, Healthcare Provider CPR and AED Certification within 3 months of hire.
- Possesses the training, knowledge, and skills necessary to perform one or more of the following activities (evaluation, treatment, and/or patient care) for patients in one or more of the following age groups: neonatal, pediatric, adolescent, adult, and geriatric.
- Ability to effectively communicate and interact with employees, patients, and family members in groups and on a one-to-one basis.
- Ability to problem solve.
- Familiarity with a variety of equipment, including computer software, is required.
- Organization, time management, flexibility, and positive attitude are crucial to successful performance.
- Continuous professional development is expected.
- Current License in the State of Wisconsin as Speech Language Pathologist at the time of hire or prior to providing services.
- Must be able to organize and prioritize multiple demands.
- Maintain ethical conduct and keep confidential personal and medical information about patients.
- Must adhere to established hospital and Physical Medicine departmental policies, rules and regulations.
- Services are provided in compliance with federal, state, professional organization.
- Reporting relationship Director of Physical Medicine and Rehabilitation and Clinical Coordinator.
