We are seeking a compassionate and dedicated
Social Worker/
Outpatient Mental Health Therapist to join our Behavioral Health team. In this role, you will provide assessment, counseling, and therapeutic interventions to individuals, families, and groups experiencing mental health, emotional, behavioral, or psychosocial challenges. The ideal candidate is passionate about helping patients achieve their mental wellness goals and is committed to providing high-quality, patient-centered care.

  • Conduct comprehensive psychosocial assessments and evaluations
  • Develop individualized treatment plans based on patient needs, goals, and clinical findings.
  • Provide evidence-based individual, family, couples, and group therapy.
  • Monitor patient progress and adjust treatment plans as appropriate.
  • Assess and manage risk factors, including suicide ideation, self-harm, abuse, and crisis situations.
  • Maintain timely and accurate documentation in the electronic medical record (EMR).
  • Collaborate with psychiatrists, primary care providers, case managers, and other healthcare professionals to coordinate patient care.
  • Educate patients and families regarding diagnoses, treatment options, coping strategies, and community resources.
  • Participate in case consultations, team meetings, and quality improvement initiatives.
  • Comply with all regulatory requirements, ethical standards, confidentiality guidelines, and organizational policies.
  • Participates in an on-call rotation (1-2 weeks per year)
  • Maintain knowledge of current behavioral health treatment practices and participate in continuing education opportunities.
Job Requirements
  • Master's degree in Social Work, Counseling and Guidance or related degree from an accredited university
  • Ideally Licensed as LPC, LCSW or LMFT in Wisconsin (Minimal License is LPC-IT or MSW-IT)
  • Ideal candidate will have 3000 hours' post-graduate experience

    Ability to establish and maintain therapeutic relationships with patients and families
  • Strong clinical judgment, problem-solving, and crisis intervention skills.
  • Ability to work independently while collaborating effectively within an interdisciplinary team.
  • Excellent organizational, documentation, and time management skills.
  • Commitment to providing culturally competent and patient-centered care.
  • Ability to maintain confidentiality and professional boundaries.
  • Excellent verbal and written skills
  • Experience with electronic medical record (EMR) is a plus.
