JOB SUMMARYThe Nurse Practitioner / Physician Assistant provides professional nursing and medical services to individuals, families, and groups with an emphasis on health promotion and disease prevention, according to their advanced practice training. Provides diagnosis and management of acute and chronic conditions within their scope of practice, following evidence-based guidelines, CHS clinical policies, and Wisconsin state regulations.
ESSENTIAL JOB FUNCTIONS- Obtains complete health history and appropriate assessments on patients.
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.
- Orders appropriate laboratory and diagnostic procedures.
- Maintains and reviews patient records, charts and other pertinent information.
- Participates in educating patients and families on their preventive care, medical issues, and use of prescribed medications and treatment.
- Documents all patient encounters in the electronic health record (EHR) in accordance with CHS policies, payer requirements, and state and federal regulations; completes documentation within required timelines. Follow up on all tests ordered.
- Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
- Prescribes and administers medications according to established protocols and standards.
- Participates in patient care conferences and the formulation of individual and family care plans.
- Provides follow-up care for patients throughout the course of a study.
- Interviews and advises patients regarding health and illness prevention.
- Recommends community resources to meet patient and family needs.
- Participates in the development and implementation of health care policies, procedures, and standards of care.
- Participates in the evaluation of the Health Care Plan, the care given and the clinical outcome evaluations.
- Participates in clinic staffing and Quality Assurance Committee meetings and quality assurance and peer review on a periodic basis.
- Attends required meetings. Supports health education for patients during clinical encounters; collaborates with CHWs, nursing staff, and care teams for broader outreach or education initiatives as needed.
- Participates in community health education programs and self-directed continuing education.
- Works in conjunction or in collaboration with medical providers.
- Complies with CHS clinical protocols, HRSA requirements, FTCA guidelines, and organizational quality and safety policies.
- Completes required trainings on time.
- Performs clinical procedures appropriate to training and credentialing, such as joint injections, skin biopsies, cryotherapy, suturing, abscess I&D, Mirena/IUD procedures (if trained), and other minor procedures commonly performed in primary care.
- Other duties as assigned.
- Participates in the clinic-wide Rapid Response system; responds to Rapid Response activations in Medical, Dental, and Behavioral Health according to the assigned rotation and clinic policy. Participates in walk-in/acute care access as determined by the Medical Director.

- Ability to relate well to individuals and families of low-income, multi-cultural neighborhoods, recognizing their right to participate in health care planning and decision-making.
- Ability to understand patient attitudes and needs, develop and orient program goals and concepts accordingly, and to work effectively toward accomplishment of these goals with all health care team members.
- Ability to establish and maintain effective working relationships with government, or organizational officials, the public, patients, representatives of private and public health organization, health care personnel, and community groups and organizations.
- Ability to work under pressure in a fast-paced environment, maintain personal composure, evaluate situations and act/react appropriately in dealing with difficult situations.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Ability to communicate orally and in writing, information/presentations with a wide variety of clients and staff.
- Ability to read and respond to in-box messages and emails in a timely fashion.
- Ability to identify and maintain confidential/HIPPA information.
- Masters of Nursing, Doctorate in Nurse Practice, Masters of Physician Assistant studies from an accredited school.
