Beloit Health System is hiring a Licensed Practical Nurse to join our team at Beloit Regional Hospice.

We are currently seeking a dedicated and adaptable Licensed Practical Nurse (LPN) to join our Beloit Regional Hospice team.

Job Overview

The primary responsibility of the Hospice LPN is to support the day‑to‑day activities of the hospice staff and clinical services, including clinical and clerical duties. The LPN provides information and services as a resource to others, achieving defined agency objectives in compliance with established guidelines. The focus is to facilitate the highest quality of care through comprehensive and consistent support.

Key Responsibilities
  • Exhibit each of the Beloit Health System Behavior Standards and role model excellence for all to see.
  • Provide telephone calls and patient triage support.
  • Supply and order DME (durable medical equipment).
  • Complete audits, documentation review, and related follow‑up.
  • Complete patient visits as needed.
  • Assist with and participate in marketing opportunities.
  • Support RN case managers with patient needs and follow‑up to phone calls.
  • Assist clinical manager with staff training, competencies, meeting minutes.
  • Provide coverage for palliative care during vacation and illness.
Qualifications
  • Graduate of an accredited practical nursing program.
  • Active Wisconsin LPN at time of hire.
  • Illinois LPN preferred; if not active IL at hire, must apply for IL license within 30 days of hire.
  • 2 years’ experience in palliative care or hospice preferred.
  • Demonstrated ability to interact with a wide variety of medical, professional and community groups.
  • Computer proficiency and strong public speaking skills.
  • Valid driver's license.
  • Auto insurance coverage meeting requirements and maintaining a reliable vehicle.
  • Ability to work independently and make autonomous decisions.
  • Current CPR certification required.

Reporting Relationship:
Clinical Manager and Department Director.
