General Description

Serves as the Lead Crisis Intervention Specialist for IDD Services. Works collaboratively with Service Coordinators, providers, hospitals, law enforcement, and other agencies to identify individuals with intellectual and developmental disabilities (IDD) who are at risk of crisis. Provides consultation, education, and intervention strategies to prevent crisis events and support stabilization. Coordinates crisis respite services, including admissions, discharge planning, and therapeutic support planning.

Ensures compliance with all HHSC Performance Contract requirements and maintains accurate and timely documentation. Services are provided in a variety of settings including, but not limited to: client homes (family, group, or host homes), community settings, CCS clinics, day habilitation sites, and crisis respite programs.

Essential Duties and Responsibilities Assessment:
  • Respond to referrals for Crisis Intervention Services within 48 hours.
  • Complete initial screenings/assessments to determine level of need and appropriate services.
  • Conduct ongoing reassessments to evaluate progress and need for continued services.
Education, Training, and Consultation:
  • Provide education on IDD services to individuals, families, and providers.
  • Deliver consultation to Mobile Crisis Outreach Teams (MCOT), law enforcement, emergency departments, and community partners.
  • Provide client specific education and facilitate training sessions for caregivers and support systems.
  • Identify and implement prevention strategies to reduce crisis events and promote coping skills.
Treatment Planning:
  • Gather information from clients and stakeholders to develop therapeutic support plans.
  • Identify supports necessary for successful community placement.
  • Complete discharge planning for crisis respite services.
Crisis Respite Coordination:
  • Coordinate admission and discharge for all crisis respite services.
  • Maintain documentation and ensure all required paperwork is complete prior to service delivery.
  • Collaborate with Behavior Support Supervisor and providers to coordinate services.
  • Provide support and training to respite staff and contracted providers.
  • Maintain master schedules and coordinate appointments during respite stays.
  • Serve as liaison between the crisis intervention team and the individual’s support system.
  • May provide direct care as needed, including implementing therapeutic strategies and supporting daily living needs.
Placement and Coordination:
  • Collaborate with Placement Coordinator on identified cases.
  • Submit diversion requests and coordinate referrals to Transition Support Teams.
  • Participate in CRCG meetings as needed.
Documentation:
  • Complete all assessments, plans, and progress notes in a timely manner.
  • Enter progress notes within 48 hours of service delivery.
  • Provide updates to referral sources regarding service status.
Administrative Tasks:
  • Complete required HHSC reporting accurately and timely.
  • Maintain provider networks for crisis respite services.
  • Coordinate with IDD staff to ensure proper documentation for billing.
  • Process purchase orders for contracted providers.
  • Assist leadership with budgeting, program evaluation, and service improvements.
