- Provide high-quality home health care to patients in their preferred home setting
- Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs
- Assume primary responsibility for the caseload, including assessing, planning, coordinating, implementing, and evaluating treatment plans
- Assess physical, psychological, social, and spiritual factors influencing patient, family, and caregiver health status and incorporate data into the plan of care
- Conduct initial evaluations and re-evaluate patients’ nursing needs during each visit
- Make follow-up visits according to patient, family, and facility needs
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other care personnel
- Revise plans of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members
- Align all activities with Elara Caring’s vision and Home Health leadership
- Report to the Clinical Manager or Clinical Supervisor
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Associate’s Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice experience is preferred
- Previous experience with Home Care Home Base is preferred
- Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
Demonstrates expertise in patient assessment, care planning, and coordination in home health settings, with a strong focus on maintaining patient privacy and compliance with HIPAA regulations.
