Under limited supervision, the Community Services Representative III (Care Case Manager) is responsible for implementing program activities for the Minority AIDS Initiative Medical Case Management Program.
THIS IS A LIMITED TERM, GRANT FUNDED POSITION TO FEBRUARY 28, 2010. CURRENT AND CONTINUED EMPLOYMENT BASED UPON AVAILABLE FUNDING.Bilingual English/Spanish Is Required.
Essential FunctionsPerforms outreach activities to identify potential HIV/AIDS and other at‑risk clients for targeted services; conducts psychosocial assessments; develops treatment plans; provides referrals and linkages to services; treatment advocacy; maintains current up‑to‑date progress notes; implements individual treatment plans; collects data and reports activities utilizing Case Watch reporting and collection systems; participates in program evaluation activities, clinical supervision, and case conferences; establishes and maintains active relationships with agencies that are potential sources of client recruitment and resources;
maintains regular and active presence at various community meetings representing the City of Pasadena; serves as the lead on issues related to client care services; assists clients with the ADAP enrollment process and remaining adherent and compliant with medication regimens and medical appointments; performs related duties as assigned.
Any combination of education, training, and experience which has provided the necessary knowledge, skills and abilities for acceptable job performance, such as: three years of experience providing community service in the area of HIV/AIDS case management, treatment advocacy, education, and/or clinical services. A bachelors' degree in Social Work, Psychology, Sociology, or other related field is highly desirable. Certified HIV Test Counselor and/or ADAP enrollment worker are a plus.
Bilingual English/Spanish is required.
Skills And Abilities
Knowledge of the risk factors and behaviors associated with HIV/AIDS, human sexuality, substance abuse, sexually transmitted diseases, mental health issues, and domestic violence; AIDS Drug Assistance Program and community resources; possession of strong social and/or cultural ties within the targeted populations and demonstrated cultural competence; MS Office programs, including Internet and Case Watch Reporting System. Ability to work with individuals suffering from chronic illnesses, substance abuse, mental disorders, and homelessness;
culturally and ethnically diverse populations; professionally represent the department in community meetings and outreach events; work independently with minimal supervision; communicate effectively both verbally and in writing; maintain effective and professional relationships with clients, staff, and members of the community; develop and maintain accurate written documentation; bilingual in English/Spanish.

Probationary work test period is six months.
VACANCIESThere is currently one Limited Term, Grant Funded vacancy in the Public Health Department, which may be filled as a result of this examination process.
Special RequirementsPossession of or ability to obtain a Class C California driver's license, access to a car and proof of automobile insurance to travel to different work sites. Must be available to work some weekends and evenings as necessary. Bilingual English/Spanish is required.
FLSA - Non-ExemptEffective January 1, 2013, new members to CalPERS or an agency with CALPERS' reciprocity or who have more than a six month break in service between employment in a CalPERS (or reciprocal) agency will be subject to the provisions of the Public Employees' Pension Reform Act of 2013 (PEPRA) and will receive the 2% @ 62 benefit formula.
Employees who are current members of CalPERS or an agency with CalPERS' reciprocity, or who have less than a six month break in service between employment in a CalPERS (or reciprocal) agency and employment with the City will be enrolled in the 2.5% @ 55 benefit formula.
All employees are required to pay the full employee contribution to CalPERS.
