MDS Coordinator

Sapphire at Northwoods is seeking an MDS Coordinator to assist the Director of Nursing while ensuring that documentation meets federal, state, and certification guidelines.

Responsibilities:

• Assures completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.
• Initiates and monitors RAI process tracking, discharge/reentry and Medicaid tracking forms through the Point Click Care system.
• Oversees functions related to MDS 3.0 regulatory and financial including scheduling, ensuring timeliness in completion and submission, CAA and comprehensive care plan, reviewing and correcting validation reports, monitoring significant changes and IPA.
• Follows up with staff to assure compliance with documentation standards.
• Completes patient assessments, data collection, and interviews staff to assure standard of practice as instructed in the current version of MDS User’s Manual.
• Facilitates accurate determination of the Assessment Reference Date that reflects the patient’s care needs and captures resources for appropriate payment by Medicare/Medicaid and insurance programs.
• Provides interdisciplinary schedules for all MDS assessments and care plans as required by OBRA and PPS.
• Assures that appropriate signatures are obtained as required.

Qualifications:

• Graduate of an approved nursing program and licensed in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred. Excellent knowledge of Case-Mix, the Federal Medicare PPS process, Medicaid reimbursement, Quality Indicator process, OBRA regulations, and the Minimum Data Set. Knowledge of the care planning process.
