Residential Program Case Manager

The Residential Program Case Manager is responsible for coordinating and overseeing case management duties for clients in a residential treatment setting. This role involves conducting client and family interviews, maintaining communication with the treatment team, and ensuring that the client's daily needs are met in a safe, supportive, and therapeutic environment. The Case Manager will collaborate with a multidisciplinary team to develop and implement treatment plans and ensure compliance with state and agency guidelines.

This role also requires effective communication with clients, families, and other referral sources to facilitate the progress and well-being of each client.

Key Responsibilities

Coordination and Communication

  • Ensure program compliance and maintain a positive therapeutic milieu for residents.
  • Manage all aspects of family coordination, including coordinating home visits and regular communication with family members and referral sources.
  • Ensure that all necessary paperwork, including releases and consent forms, are completed and signed by the appropriate parties.
  • Obtain and review background records to help develop a comprehensive client history.
  • Schedule and coordinate appointments for clients, including psychological, psychiatric, medical, dental, chemical dependency assessments, court hearings, and intake/discharge staffings.
  • Arrange transportation for client appointments and ensure that clients' basic needs (e.g., clothing, hygiene products, medications) are met.
  • Maintain continuous communication with the treatment team, including the Clinical and Program Directors, to ensure coordinated care and progress.
  • Collaborate with the treatment team to formulate the most appropriate recommendations and interventions for each client.

Program and Facility Upkeep

  • Ensure that the Residential Program is maintained in a clean and sanitary condition.
  • Contribute to creating a positive and therapeutic living environment that fosters growth and development for the youth in the program.

Licensing and Compliance

  • Ensure that all program and client-related activities are compliant with group residential requirements and relevant state rules.
  • Develop and implement a file checklist to ensure the accuracy and compliance of all client files and documentation.
  • Conduct periodic reviews of client files to ensure they are up-to-date, complete, and compliant with licensing rules and regulations.
  • Observe day-to-day unit operations to ensure that the Residential Program adheres to state rules, agency policies, and procedures.
  • Perform random file audits as required for quality assurance purposes.

Resident Interaction and Counseling

  • Engage in daily interactions with each resident to monitor their progress and address any immediate needs or concerns.
  • Interview clients, families, and collateral sources to understand the reasons for placement and gather information on the client's social history.
  • Assess current strengths and needs and gather relevant information to inform the client's treatment planning.
  • Collaborate with the treatment team to develop individualized treatment plans, ensuring that goals are aligned with client needs and progress.

Documentation and Reporting

  • Conduct bi-weekly reviews of client documentation to ensure accuracy, completeness, and timely updates.
  • Develop and update individualized treatment plans in collaboration with the Program Director and treatment team.
  • Ensure all required paperwork, including progress notes, treatment plans, and other documentation, is completed in accordance with agency policies and regulatory standards.
  • Maintain file compliance according to state and agency rules, including accurate and thorough documentation in client records.

Public Relations and Family Coordination

  • Maintain weekly communication with referral sources and family members to provide updates on client progress and address any questions or concerns.

Meetings and Professional Development

  • Attend weekly Clinical Meetings and participate in monthly training to stay updated on best practices and developments in the field.
  • Participate in shift exchange meetings to discuss client progress and ensure continuity of care.
  • Organize and facilitate admissions and discharges in collaboration with the treatment team.
  • Attend weekly clinical supervision and ensure that treatment plans and therapeutic interventions remain client-centered and responsive to evolving needs.

Working Conditions

  • This position is based in a residential setting, where you will interact with clients who may display a range of behavioral and emotional challenges.
  • The work environment requires flexibility and the ability to adapt to the evolving needs of clients and the program.
  • The position may require working evenings or weekends, depending on client and program needs.
  • Exposure to emotionally charged situations; the role requires the ability to manage stress and practice de-escalation techniques when necessary.
