Primary Responsibilities
- Provides high quality clinical services within the scope of practice as defined by the state laws governing nursing, in accordance with the plan of care and in coordination with other members of the health‑care team, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record, utilizing the agency’s electronic visit verification program (as applicable).
- Actively participates in the agency’s care‑coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management staff, case coordinators, and community case managers (as applicable).
- Acts in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- Current and unrestricted LPN licensure in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Current CPR certification or ability to complete it within 90 days of hire.
- Home‑care experience.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Good communication, writing, and organizational skills.
- Ability to work flexible hours as required to meet identified client needs.
Annual Target Pay: $51,738 to $77,606.
Hourly Pay Range: $24.87 to $37.31.
Per Visit Point Pay Range: $29.85 to $44.77.
Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity and generally follows the ranges above. Total cash compensation includes earnings from per visit point pay and hourly pay, based on factors such as local labor markets, education, and work experience, and may increase over time based on productivity and performance. The role receives two types of compensation: per visit point rate for visits and hourly rate for non‑visit activities such as orientation.
