This position supports students with mental and behavioral health needs as they transition from Day Treatment back to their home school and community. Working closely with students, families, community agencies, and school staff, the role coordinates transition planning, enrollment, and student records to ensure continuity of services and educational support. As part of a collaborative school team, this position plays an important role in helping students successfully continue their educational progress following their time in the Long Term Care and Treatment program.

• Coordinate student transition and reentry planning from the Long Term Care and Treatment (LTCT) program to receiving school districts and educational programs.
• Develop and maintain collaborative relationships with students, families, school district staff, treatment providers, and community agencies to support successful student transitions.
• Schedule and facilitate transition meetings and assist students and families in navigating reenrollment, placement options, and educational services following discharge.
• Identify barriers to student reentry and collaborate with partners to support student access to appropriate educational programs and services.
• Collaborate with LTCT educational and treatment staff to ensure coordinated transition planning and service delivery.
• Maintain accurate, complete, and confidential student enrollment, withdrawal, transition, and educational records in compliance with FERPA, HIPAA, and applicable state and federal laws, regulations, and agency requirements.
• Coordinate the transfer of student records between LTCT and receiving school districts or educational programs and organize records and data in accessible formats to support communication and decision‑making.
• Maintain and update student information within Synergy and other required databases.
• Collect, track, analyze, and report data related to student transitions, educational outcomes, and program effectiveness.
• Prepare and provide reports to LTCT leadership, partner school districts, and other stakeholders as requested.
• Promote equity, inclusion, and anti‑racist practices in all aspects of service delivery and professional interactions.
• Demonstrate professionalism, effective communication, collaboration, and teamwork while working with students, families, colleagues, school districts, and community partners.
• Prioritize responsibilities, meet deadlines, maintain regular attendance, and perform work independently with minimal supervision.
