Overview
The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.
Responsibilities- Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including advising physicians on drug therapy, drug toxicity, and side effects, and assisting in prescribing appropriate doses.
- Coordinate the activities of assigned pharmacy technicians engaged in preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians’ prescriptions. Ensure compliance with federal, state and local laws and regulations pertaining to dispensing drugs and controlled substances and the maintenance of required records.
- Mentor and train others in training, including practical experiences.
- Maintain liaison relationships with medical and nursing staff. Provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.
- Conduct and evaluate medication histories, assess compliance and suggest modifications to achieve desired outcomes; instruct patients in the proper use of prescribed drugs and participate in Patient Care Rounds with physicians to evaluate patient progress.
- Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations, and contribute to establishing and improving the delivery of pharmaceutical care within areas of direct responsibility, assisting others in the department.
- Extract pertinent information from patient and other sources for the patient’s medical management plan.
- Role model two-way communication of the organization’s mission, vision, values, and strategic plan for the department.
- Support the organization’s vision, mission and values in all respects and promote Lean principles of continuous improvement.
- Provide and maintain a safe environment for caregivers, patients and guests.
- Perform activities with high standards of professionalism and confidentiality. Comply with laws, regulations, policies and procedures; uphold corporate integrity and report known or suspected violations; cooperate with investigations.
- Deliver customer service and patient care that is timely, efficient and accurate.
- May perform additional duties of similar complexity within the organization, as required or assigned.
- Required
:
Bachelor of Science degree in Pharmacy. - Preferred
:
Pharm
D., Completion of an ASHP accredited PGY-1 residency training.
- Required
:
Current registration by the Oregon Board of Pharmacy. AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy). - Preferred

Any certification through the Board of Pharmacy Specialties (BPS).

Six (6) months or more clinical pharmacy experience.
