Overview
Brookdale provides dementia care, including adapted ADL’s, meaningful engagement, and communication to residents following a person‑centered individual service plan. Residents receive bathing, dressing, grooming, toileting, and transferring assistance, as well as help with meals and activities. The role supports a purposeful, engaging culture and fosters independence and dignity for residents.
Responsibilities- Assist residents with activities of daily living (ADL) such as bathing, dressing, grooming, toileting, and transferring, following the individualized intervention plan.
- Support residents in program participation, meals, and necessary transportation in accordance with the resident’s service plan.
- Provide meaningful engagement through daily activities, prompting, cueing, and staff–led programs.
- Record and report changes in residents’ eating habits, medication administration (where permitted), and overall health or behavior to the supervisor.
- Maintain resident records daily, ensuring accuracy and timeliness in accordance with company policies.
- Document medication distribution, incidents, observations, and other relevant information; assist with medication administration only if authorized by state regulations.
- Perform general housekeeping tasks, supporting resident laundry, bedroom, dining, and other areas per cleaning schedules.
- Respond promptly to emergency situations and resident requests for assistance (e.g., pull cords, phone calls).
- Monitor resident location and notify supervisors if residents cannot be located.
- Encourage teamwork and cooperative interactions with co‑workers.
- Follow physician orders and state laws for medication administration and related procedures.
- High school diploma or GED required; three to six months of related experience preferred.
- Training or direct service experience with older adults living with dementia preferred.
- Knowledge of dementia, especially Alzheimer’s disease.
- Basic communication skills in English with residents, families, and associates.
- Ability to read, comprehend, and write simple correspondence.
- Provides strong judgment in emergency situations and can seek assistance when needed.
- Veterans, transitioning active‑duties military personnel, and military spouses encouraged to apply.
- Preferred:
Current state certification required by law for medication duties.
- Standing, walking, sitting, reaching, climbing balance, stooping, kneeling, crouching, or crawling.
- Use hands and fingers to handle or feel, lift items up to 50 pounds.
- Requires good vision, hearing, taste, and smell.
- May involve occasional weekend, evening or night work; on‑call availability as needed.
- Exposure to communicable diseases, blood‑borne pathogens, latex, chemicals, and other biological hazards.
- Potential for injury from falls, burns, cuts from equipment, or transfers.
- Occasional travel required.
- Current state certification may be required; must be maintained per regulations.
- Ability to follow detailed guidelines; moderate independent judgment for routine decisions.
- Understand and comply with state regulations for direct care and medication duties.
