The St. Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use.
The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.
Essential functions and duties include:
- Providing specialized pharmaceutical services to improve drug usage and therapeutic outcomes
- Coordinating the activities of assigned pharmacy technicians
- Mentoring and training others in training
- Maintaining liaison relationships with medical and nursing staff
- Conducting and evaluating medication histories
- Individualizing medication regimens using sound principles
- Extracting pertinent information for the patient's medical management plan
- Role modeling two-way communication of the St. Charles Health System mission, vision, values, and strategic plan
- Supporting Lean principles of continuous improvement
- Providing and maintaining a safe environment for caregivers, patients, and guests
- Conducting all activities with the highest standards of professionalism and confidentiality
- Delivering customer service and/or patient care in a manner that promotes goodwill, is timely, efficient, and accurate
Education:
- Required:
Bachelor of Science degree in Pharmacy - Preferred:
Pharm
D., Completion of an ASHP accredited PGY-1 residency training
Licensure/Certification/Registration:
- Required:
Current registration by the Oregon Board of Pharmacy. AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy) - Preferred:
Any certification through the Board of Pharmacy Specialties (BPS)
Experience:

Six (6) months or more clinical pharmacy experience
Personal Protective Equipment:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Standing and walking, carrying 1-10 pounds, operation of a motor vehicle for caregivers that are couriers. - Frequently (50%):
Lifting 1-10 pounds, pushing or pulling 1-25 pounds, grasping/squeezing. - Occasionally (25%):
Sitting, bending, climbing stairs, lifting and carrying 11-25 pounds - Use of clear and audible speaking voice and ability to hear normal speech level.

Keyboard operation, stooping/kneeling/crouching, reaching overhead, pushing or pulling 25-50 pounds. - Never (0%):
Climbing ladder/stepstool, lifting or carrying 25-50 pounds, whispered speech level, operation of a motor vehicle. - Exposure to Elemental Factors
