Behavioral Health Navigator
The Behavioral Health Navigator (BHN) is committed to removing barriers to care by identifying critical resources for patients and helping them navigate through behavioral health and substance use disorder services in the Central Oregon region while promoting patient health. The Behavioral Health Navigator will support the behavioral health department by supporting administrative tasks and assisting in quality improvement projects.
Responsibilities include operating as an extension to the primary care behavioral health team, coordinating care for patients both within and outside of the clinic, following up on external referrals for behavioral health care, scheduling and coordinating trainings, attending community partnership meetings, and attending to behavioral health quality improvement initiatives.
Patient support includes coordinating care for patients, creating optimal experiences for patients and families, facilitating care coordination, connecting with internal partners, monitoring behavioral health and substance use disorder pools, communicating with patients to schedule appointments, and utilizing effective practice management skills.
Education, certification, and licensure require a high school diploma or GED, with a bachelor's degree in a related field preferred. Skills and knowledge include understanding behavioral health program services, competencies in the PCBH program manual, foundational understanding of population-based services, skills in organization, computer skills, documentation, clear communication, knowledge of EPIC, managing referrals and phone calls, time management skills, and practice management skills.
