Mosaic Community Health in Central Oregon is seeking a Behavioral Health Navigator to support the primary care behavioral health team. You will coordinate care for patients within and outside the clinic, follow up on external referrals, and assist in documenting referrals and outcomes.
Responsibilities include scheduling BH and SUD/MAT trainings, building partnerships with community organizations, and contributing to quality improvement initiatives under BH leadership.
