Deschutes County Health Services supports over 400 staff in Behavioral and Public Health programs working to promote and protect the health and safety of the community in a broad variety of community-based settings.

This position is part of Public Health service area, which works to keep people healthy and safe by striving to improve and protect the health of people in Deschutes County. ABOUT THE JOB:

Deschutes County is currently accepting applications for a Public Health Educator I – Community Health Worker for the Perinatal Care Coordination program within the Family Support Services team, which is part of Clinical & Family Services in Deschutes County Public Health. Family Support Services programs include Babies First!, CaCoon, and Perinatal Care Coordination (PCC). The nurse home visiting programs provide personal nurse visits that include comprehensive maternal and infant assessments, case management, linkage to community resources, and parent education and skills building.

Our current nurse home visiting programs are Babies First! (nurse home visiting for pregnant and post‑partum women, and children 0-5) and CaCoon (nurse case management for children 0 to 21 with special health care needs). The PCC program is designed to increase access to medical care and enrollment into available perinatal resources during the prenatal and postpartum period for individuals and their families.

The incumbent will work with external partners in the community, and pregnant women in a clinical setting providing PCC program services, referral coordination, and health promotion and education activities appropriate for the perinatal population.

This position will be an integral part of the Family Support Services team, working in conjunction with home visiting nurses and perinatal care coordinators to improve the health outcomes of the individuals and families in our community.

This is a limited‑duration, full‑time Monday through Friday, 8am-5pm position which will be based onsite at the Bend Health Services building, and will also include work in the community and at other Health Services locations throughout the county. This is not a remote work position.

Key Responsibilities:
  • Conducting clinic and telehealth visits with clients, connecting them with resources, and providing OHP enrollment assistance.
  • Providing service navigation support, care coordination, and referrals for individuals and families to community resources.
  • Documenting visit encounters to include assessment, planning, implementation, and evaluation of care in our EHR and data collecting systems.
  • Establishing good working relationships with community partners, and promoting and informing them of perinatal services at Deschutes County Health Services.
  • Managing external and internal referrals for perinatal services at Deschutes County Health Services.
