Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in the community. Visits are ~45 minutes, with a focus on patient engagement, medication and history review, and optional diagnostic testing as ordered by the health plan.
We offer a base hourly rate with incentives, malpractice coverage during Signify Health activities, and supplies provided.
