The Job

  • Provide individualized nutrition care by assessing patients, developing personalized care plans, and supporting healthy growth and development from infancy through adolescence.
  • Educate and empower families through nutrition counseling, chronic disease management, patient education, and culturally responsive, family-centered care.
  • Coordinate care across the healthcare continuum by partnering with primary care providers, specialists, community organizations, and social service agencies to ensure seamless patient support.
  • Advocate for patients by assisting with medical necessity documentation, insurance appeals, nutritional equipment and formula requests, and connecting families with needed resources.
  • Drive quality and collaboration by participating in interdisciplinary care teams, quality improvement initiatives, care coordination, and Patient-Centered Primary Care Home (PCPCH) efforts while promoting evidence-based pediatric nutrition.

Requirements

Education and Certification

  • Bachelor's Degree in Dietetics, foods and nutrition or related area granted by a U.S. regionally accredited college or university.
  • Master's degree required.
  • Current Oregon Registered Dietitian Licensure in good standing required.
  • Additional recognized certifications in addition to RD (required) include:
    Certified Diabetes Educator, OHA HCI Qualified or Certified Interpreter.

Experience

  • Preferred experience in a healthcare environment or working with young children & families experiencing specialized needs, or other high-quality early care and education settings.
  • Exceptional and proven communication skills and ability to interact with a diverse population of professionals. Ability to facilitate and reach consensus among health care professionals and community partners to achieve expected results. Ability to multi-task and work independently. Attention to detail. Strong analytical, problem solving and decision-making skills. Ability to work in a team and promote team driven outcomes. Knowledge of Patient Centered Primary Care Home and Care Coordination curriculum.

    Clinical informatics knowledge and skills. Experience giving presentations.
  • Epic EMR experience preferred. Proficient with Microsoft Office Suite.
  • Bilingual in Spanish and English a plus.
