Deschutes County Health Services is seeking a Public Health Educator I – Community Health Worker for the Perinatal Care Coordination program within the Family Support Services team in Bend. The nurse home visiting programs include Babies First!, CaCoon, and Perinatal Care Coordination (PCC).
You will provide PCC services, referral coordination, and health promotion and education tailored to perinatal populations in our community.
