Nurse Practitioner

Partners In Care, the region’s largest and longest-serving nonprofit provider of home health, palliative care, and hospice services, is seeking a compassionate

Nurse Practitioner to support our hospice patients across Central Oregon. This role focuses on required face‑to‑face visits, comprehensive assessments, interdisciplinary collaboration, and expert symptom management.

• Complete comprehensive patient assessments and documentation
• Support and mentor hospice RNs as a clinical resource
• Collaborate with physicians and the interdisciplinary team
• Provide field visits for patients needing more intensive symptom management
• Serve as PCP for some patients as appropriate
• Travel throughout the Central Oregon service area, including Bend, La Pine, Crescent, Christmas Valley, Prineville, Madras, and Sisters

• Unencumbered, Licensed Nurse Practitioner in Oregon (Family or Adult NP preferred)
• Active DEA registration
• Strong pharmacology knowledge, including controlled substances
• 3–5 years of primary care experience preferred
• Experience with medically complex or underserved populations a plus
• Hospice experience or understanding of LCD guidelines preferred
• Strong communication skills, cultural competency, and a collaborative approach
• Valid driver’s license, insurance, CPR certification, and ability to pass screenings
