Hourly Salary Range: $46.00 - $54.00

Flexible Per Diem Schedule with coverage area 60 mile radius from Bend (includes Madras, Sisters, Prineville, Redmond, Terrebonne, Crooked River Ranch, Culver, Sunriver, La Pine, Gilcrest).

• Complete an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Provide professional nursing care by utilizing all elements of the nursing process.
• Assess and evaluate patient status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan that establishes goals based on nursing diagnose.
• Include the patient and the family in the planning process.
• Record pain/symptom management and changes/outcomes as appropriate.
• Communicate with the physician regarding the patient’s needs, report changes, and obtain/receive physicians’ orders as required.
• Teach the patient and family/caregiver self-care techniques, as appropriate.
• Work in concert with the interdisciplinary group.
• All other duties as assigned.

• At least one (1) year of acute care experience in an institutional setting.
• One (1) year of experience in home care preferred.
• Current licensure in the state.
• Understanding of the philosophy of hospice concept and needs of the terminally ill.
• Flexibility in work hours.
• Demonstrated nursing skills per competency checklist.
• Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
• Ability to travel throughout the Bristol Hospice Service Area.
