Our client is currently seeking a Hospice/Home health Registered Nurse (RN).
Schedule: Full-Time | 40 Hours per Week | Monday-Friday | 8:00 AM - 4:30 PM
Mileage Reimbursement: Yes, between patient visits
On-Call Requirement: Approximately once per week
About The OpportunityWe are seeking an experienced Hospice Registered Nurse (RN) to join a compassionate hospice care team providing high-quality, patient-centered care throughout Central Oregon. This travel position offers the opportunity to make a meaningful difference in the lives of patients and families by delivering skilled nursing care and coordinating end-of-life services in the home setting.
Key Responsibilities- Manage a caseload of hospice patients and provide comprehensive case management services
- Conduct patient assessments and develop individualized care plans
- Coordinate care with physicians, interdisciplinary team members, patients, and families
- Monitor patient conditions and adjust care plans as needed
- Provide education and emotional support to patients and caregivers
- Administer medications, treatments, and symptom management interventions
- Complete accurate and timely clinical documentation
- Participate in on-call rotation and respond to patient needs as required
- Ensure compliance with hospice regulations, policies, and standards of care
- Travel throughout the designated coverage area to provide in-home patient care
- Active Oregon Registered Nurse (RN) License
- Current BLS Certification
- Recent Hospice Nursing Experience
- Strong case management skills
- Valid driver's license and reliable transportation
- Ability to travel throughout the assigned territory
