** Position Summary*
* We are seeking an experienced  
** Travel Registered Nurse (RN)
** to provide compassionate hospice care across Central Oregon. The RN will be responsible for  
** hospice case management** , coordinating patient care, monitoring patient needs, and collaborating with patients, families, and the interdisciplinary care team.

** Key Responsibilities*
* + Manage an assigned caseload of hospice patients.

+ Provide comprehensive, patient-centered hospice nursing care.

+ Assess patient conditions and monitor changes in health status.

+ Develop, implement, and update individualized plans of care.

+ Coordinate services with physicians and the interdisciplinary hospice team.

+ Communicate effectively with patients, families, caregivers, and healthcare professionals.

+ Complete accurate and timely clinical documentation.

+ Meet an expected productivity level of

+ Travel throughout the assigned coverage area between patient visits.

+ Participate in  
** on-call coverage once per week** .

+ Mileage between patient visits is  
** reimbursed** .

** Required Qualifications*
* + Current  
** Oregon RN license** .

+ Current  
** BLS certification** .

+ Previous  
** hospice nursing experience required** .

+ Strong clinical assessment and case-management skills.

+ Ability to work independently and manage multiple patient needs.

+ Excellent communication and organizational skills.

** Preferred Qualifications*

Experience with  
** Homecare Homebase (HCHB)** .

+ Previous community-based or home health/hospice experience.
