Practice Overview
- Join a team of dedicated professionals including 1 physician and 2 nurse practitioners
- Engage in a comprehensive care model focusing on chronic disease management and Suboxone treatment
- Enjoy autonomy with support from a robust partnership and Medical Home model
- Benefit from low‑volume call duties and no hospital or OB requirements
Schedule
- Flexible 40‑hour workweek with extended hours available twice a week
- Gradual ramp‑up period with extended appointment slots for complex cases
- Shared phone call duty with a low call volume, taken in 1‑week increments
- Balance between clinic and procedural time with onsite support and facilities
Compensation & Benefits
- Salary range dependent on professional experience
- Negotiable sign‑on bonus and relocation assistance in line with regional standards

Candidate Requirements
