United Counseling Service of Bennington County seeks a Clinical Care Coordinator to support clients across behavioral health, developmental, and substance use programs. In this role, you will coordinate assessments, care plans, and referrals, ensuring timely access to services for children, adults, and families. You will facilitate multidisciplinary team meetings, track outcomes, and maintain accurate clinical and administrative documentation. The position involves close collaboration with community partners, advocacy for client needs, and support for crisis and transition planning in a mission-driven environment.

Responsibilities

  • Coordinate client assessments, care plans, and service referrals across UCS programs
  • Facilitate multidisciplinary team meetings and case conferences
  • Monitor client progress and adjust care plans in collaboration with clinicians
  • Maintain accurate, timely clinical and administrative documentation in EHR systems
  • Support crisis and transition planning, including safety and discharge coordination
  • Collaborate with community partners, providers, and families to ensure continuity of care
  • Educate clients and families about available services and community resources
  • Track service utilization and outcomes to support quality improvement
  • Ensure compliance with agency policies, confidentiality, and regulatory standards
  • Participate in team meetings, trainings, and initiatives to advance person-centered care
