The Spoke Practice Social Worker provides social work services to primary care patients in participating Vermont Blueprint for Health Spoke Program and participate as an active member of the Community Health Team.
Requirements:
Two (2) years social work experience in a community, home health, or outpatient practice setting required. Knowledge of a variety of local, state, and federal resources required. Expertise in advanced care planning strongly preferred. Experience with interventions with adults, families, and children strongly preferred. Valid license in social work, vocational rehabilitation, substance abuse counseling, or related field required. Valid Driver's License and Auto Insurance required.
