A Day in the Life of a Hospice Registered Nurse (RN)

As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. This position is responsible for serving as case managers and for providing individualized goal directed nursing care through the use of the nursing process in accordance with physician's orders. Responsibilities include but are not limited to the following:

Essential functions
  • Utilizing the nursing process, assesses patient/caregiver needs in order to provide quality care to assigned patients.
  • Develops and communicates a realistic plan of care for each patient.
  • Serves as Case Manager for assigned patients and actively manages case loads and visit times in accordance with agency productivity standards.
  • Implements medical plan of care, nursing interventions, and procedures as necessary to ensure the safe, effective, quality of care for patients.
  • Stays up-to-date on a variety of high-tech skills; IV use and maintenance, wound VAC, complex wound care, etc.
  • In accordance with LPN scope of practice, works collaboratively with LPNs to ensure that the plan of care is followed, delegates tasks and ensures that patient needs are met.
  • Consistently reviews documentation, annotates visit notes and acts on recommendations in accordance with department policies and procedures.
  • Evaluates aspects of the implemented plan of care including problems, care provided and patient responses to meet patient/caregiver needs and ensure overall quality of care.
  • Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure ongoing and continued quality of care.
  • Performs patient and caregiver education to support informed patient decisions, including medication side effects, treatment options, discharge instructions, reportable conditions, and ranges of services.
Qualifications for a Registered Nurse (RN)
  • Registered Nurse (RN) license
  • 2+ years of nursing experience is preferred
  • On-call and holiday rotation required
  • Have self-direction and initiative, and be able to work collaboratively with colleagues within the agency
  • Experience with electronic medical records required
  • Willingness to work a flexible schedule including evenings, weekends, and on call for both hospice and home care patients
  • Exceptional bedside manner and communication skills
  • Compassionate and friendly demeanor
  • Valid Vermont Driver's License and car insurance
