About the Position
Job Opportunity – Registered Nurse (RN) – Home Health & Hospice – Vermont

Schedule:

Case management with flexible visit scheduling; occasional home care visits required
Highlights:
Serve as Case Manager for assigned patients, coordinating comprehensive home health and hospice care
Utilize advanced clinical skills including IV therapy, wound VAC, complex wound care, and pain/symptom management
Complete OASIS documentation in accordance with state and federal regulations
Collaborate with interdisciplinary team members including LPNs, physical therapists, social workers, and home health aides
Participate in peer review audits and monthly case conferences to support continuous quality improvement
Electronic medical records system (McKesson) for streamlined documentation and care coordination
Requirements:
Current Vermont Registered Nurse (RN) license in good standing
Knowledge of nursing process, evidence-based practice, and home health/hospice care standards

Experience with OASIS documentation and home health regulatory compliance preferred
Proficiency in high-tech clinical skills including IV therapy, wound care, and symptom management
Strong case management, delegation, and interdisciplinary collaboration abilities
Valid driver's license and reliable transportation for home visits
