As a key member of our nursing staff, you will provide high-quality patient care and support to our residents in our Subacute Rehab wing. Responsibilities

• Provide direct patient care to residents in our Subacute Rehab wing, including assessment, planning, implementation, and evaluation of care plans
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Administer medications, treatments, and therapies as ordered by physicians
• Monitor and record patient vital signs and medical conditions
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
• Maintain accurate and up-to-date patient records
• Participate in quality improvement initiatives to enhance patient care and outcomes
