• Active VT LPN License

• Minimum 1 year experience in long-term care (SNF)

• Prior supervisory or leadership experience preferred

• Supervise and support nursing staff including CNAs

• Assist in overseeing LPN and CNA team performance during shift

• Respond to clinical changes and emergency situations

• Ensure accurate documentation and regulatory compliance

• Oversee admissions, discharges, and transfers as assigned

• Administer medications and follow physician orders

• Act as a key point of contact during overnight hours

• Maintain high standards of care as an LPN Licensed Practical Nurse
