Overview
The Health Resource Manager is responsible for providing case management services for SVMC patients.
Responsibilities- Complete intake interviews to identify a holistic plan of care; facilitate multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge
- Develop a patient transition plan, provide patient/caregiver education, and proactively advocate for patient needs and goals across the interdisciplinary team to ensure an approach sensitive to each patient and family’s needs
- Assist providers in determining the patient’s level of care in accordance with Inter Qual criteria
- Actively seek collaboration with providers to improve overall quality and efficiency of care
- Graduation from an accredited school of nursing
- 2 years of recent clinical experience required
- A Vermont RN Nursing License is required by the date of employment
- Current BLS Certificate
- Prior experience in utilization review, discharge planning and/or case management is preferred but not required

- Pay Range: $36.00-$46.00
