Responsibilities:
Provide direct patient care, including administering medications, wound care, and performing vital signs
Develop and implement individualized care plans in collaboration with interdisciplinary team members
Monitor and document patient progress, reporting any changes or concerns to the nursing staff and physician
Participate in shift reports and handovers to ensure continuity of care
Collaborate with other healthcare professionals to promote a positive and respectful environment
Maintain accurate and up-to-date patient records
Requirements:
Current LPN licensure in the State of Vermont
Ability to work effectively as part of a team
Strong communication and interpersonal skills
Professional attitude and commitment to quality patient care
