Job Summary:

Crescent Manor Rehabilitation Center, located in Bennington, Vermont, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. As a valued member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our residents and patients. Our state-of-the-art facility offers a range of services, including long-term skilled nursing care and short-term post-acute rehabilitation.

• Provide high-quality patient care, including administering medications, treatments, and therapies as prescribed by physicians
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report patient progress, identifying areas for improvement and making adjustments as necessary
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with other healthcare professionals, including nurses, therapists, and physicians, to ensure comprehensive care
• Participate in patient education and family support, promoting informed decision-making and empowering patients and families

• Current LPN licensure in the state of Vermont
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
• Ability to work various shifts, including days, evenings, and weekends
• Strong critical thinking and problem-solving skills, with the ability to prioritize tasks and manage time effectively
• Demonstrated commitment to delivering exceptional patient care and supporting a culture of compassion and respect
