Responsibilities:- Provide direct patient care, including administering medications, wound care, and performing vital signs
- Develop and implement individualized care plans in collaboration with interdisciplinary team members
- Monitor and document patient progress, reporting any changes or concerns to the nursing staff and physician
- Participate in shift reports and handovers to ensure continuity of care
- Collaborate with other healthcare professionals to promote a positive and respectful environment
- Maintain accurate and up-to-date patient records
- Current LPN licensure in the State of Vermont
- Ability to work effectively as part of a team
- Strong communication and interpersonal skills
