• Provide direct patient care to residents within the scope of an LPN, under the direction of the supervising nurse
• Administer medications and document accurately in the resident chart and electronic health record
• Monitor vital signs, observe for changes in condition, and report findings to the RN or physician
• Assist with activities of daily living and support resident safety, mobility, and comfort
• Complete routine procedures within scope, including basic wound care and dressing changes as directed
• Collaborate with an interdisciplinary team to coordinate care plans and ensure smooth shift handoffs

• Active Licensed Practical Nurse (LPN) license in good standing
• Completion of an approved LPN program
• Prior experience in long-term care or skilled nursing settings preferred
• Ability to work night shifts and weekends as assigned
• Strong assessment, documentation, and communication skills
• Capable of functioning effectively in a fast-paced, team-based environment
