Join UNITED COUNSELING SERVICE OF BENNINGTON COUNTY INC as a Case Manager in our Community & Social Services department. In this role, you will coordinate services for children and families, conduct assessments, develop individualized service plans, and connect clients with community resources. You'll collaborate with educators, caregivers, and partner agencies to support children's growth and educational success. Responsibilities

  • Provide case management services to children and families enrolled in agency programs.
  • Conduct intake interviews, needs assessments, and screenings to identify family strengths and challenges.
  • Develop, implement, and regularly update individualized service plans focused on child development and educational success.
  • Coordinate services with internal program staff, schools, childcare providers, clinicians, and community agencies.
  • Advocate for children's educational, developmental, and social-emotional needs across systems.
  • Assist families in accessing community resources such as early intervention, mental health services, housing supports, and financial assistance.
  • Maintain accurate, timely documentation of contacts, service plans, progress notes, and outcomes in accordance with agency and regulatory standards.
  • Monitor family progress, adjust services and referrals as needed, and support crisis intervention when appropriate.
  • Participate in team meetings, case conferences, and program development activities.
  • Uphold confidentiality, ethical standards, and trauma-informed, family-centered practices in all interactions.
