Case Manager

Role Overview:

The Case Manager provides community-based, family-focused support for high-risk youth at risk of out-of-home placement. Under supervision, they coordinate services, assess family needs, and connect clients to medical, mental health, and community resources.

• Service Coordination:
  Collaborate with DCF, schools, and employers; facilitate access to healthcare, mental health, and recreational programs.
• Direct Service:
  Offer intensive case management and crisis intervention, develop individualized plans, build rapport, and monitor client progress.
• Advocacy & Networking:
  Act as liaison among clients, families, community organizations, and stakeholders to ensure coordinated care.
• Team & Administrative Duties:
  Facilitate team meetings, oversee treatment plan implementation, maintain documentation, and prepare reports.

• Bachelor’s degree in social work, counseling, or related field with experience working with youth and families.
• Strong crisis intervention, communication, and relationship-building skills.
• Proficiency with electronic records, case management software, and Microsoft Office.
• Ability to assess needs, develop safety plans, and work effectively with diverse populations.
• Valid driver’s license required; experience in trauma-informed care preferred.

Includes office, community, and client home visits, requiring collaboration with multidisciplinary teams and community partners to support client well-being.
